THE TOOLKIT

One patient flyer. Three role overlays. Same color zones — so every member of your team is reading the same playbook.

PATIENT-FACING · 1 PAGE

Heart Failure Zonal Flyer

The original patient-facing flyer. A simple Green / Yellow / Orange / Red zonal map of symptoms and the exact action to take. Print, hand out — your whole family can read it in 60 seconds.

FOR NURSING · RN OVERLAY

Nursing Guidance

The same zones, mapped to the RN workflow: assess, action, educate, document. Includes SBAR scripts for MD escalation and documentation language surveyors expect.

FOR THERAPISTS · PT/OT OVERLAY

Therapy Guidance

The same zones tuned for PT / OT — vitals thresholds, when to modify therapy, when to pause, and the documentation that justifies skilled need.

FOR THERAPISTS · PT/OT OVERLAY

Therapy Guidance

The same zones tuned for PT / OT — vitals thresholds, when to modify therapy, when to pause, and the documentation that justifies skilled need.

Frequently Asked Questions

Everything You Need to Know

What makes the 4-zone version different from the standard flyer?
Most zonal flyers use three zones: Green, Yellow, Red. By the time a patient hits Red, you are already calling 911. The fourth zone gives your clinical team an earlier trigger point to intervene, document, and prevent hospitalization before it is too late.
Who is this for?
Home health and hospice agencies. The patient flyer is designed for patients and families. The overlays are built specifically for RNs, PT/OT therapists, and home health aides.
Will my HHAs actually be able to use this?
Yes. The aide overlay is written in plain language with no clinical jargon. It tells them exactly what to watch for, what to say when they call the nurse, and how to document it.
Do I need anything else to use them?
No. Download, print, and hand them out. No software, no login, no setup required. If you would like us to add your agency's logo, phone number, and branding, just reach out and we will take care of it for you.
Does this replace my agency's existing heart failure protocol?
No. It works alongside your existing protocol. Think of it as the frontline communication tool that makes your protocol visible to every person in the home. If you want it customized to your agency, reach out and we will be happy to incorporate your protocol directly into the document so your team stays compliant.
What happens after I download?
You get the toolkit. A few days later you will hear from us about how HOP can train your staff on these tools so the flyers become clinical habit, not just paper on a clipboard.
